If you catch your reflection in a store window and do a double take, not because you look bad, but because you don’t quite look like you anymore, you are not alone. It is one of the most common things patients tell Dr. Gregory Keller when they sit down for a consultation at his Santa Barbara or Los Angeles office.
Do you recognize this feeling?
You look in the mirror in the morning and think you look tired, even when you slept fine. Sometimes you catch yourself looking angry when you’re not angry at all, your jawline has softened, your neck doesn’t match how you feel inside. So you reach for more concealer, more contour, more effort just to look like you did five years ago. Some days it works. Other days the makeup only settles into the lines and makes you feel older, not younger.
Little by little, it changes how you move through the world. You avoid the unflattering side in photos. You catch yourself talking with your chin tucked down. You wonder if people are noticing the sag before they notice you. For a lot of women, it is not about vanity. It is about no longer feeling like the person they know themselves to be.
If any of that sounds familiar, it is worth knowing there is a solution that does not require you to look “done,” just refreshed, rested, and like yourself again.
What a deep plane facelift actually is
Most people have heard of a facelift. Fewer people understand that not all facelifts are the same procedure.
A traditional or “SMAS” facelift lifts the skin and a superficial layer of muscle, then pulls it tighter and trims the excess. It can help, but pulling skin sideways is also why some facelifts leave people looking wind-swept, or “pulled,” rather than simply younger.
A deep plane facelift works differently. Instead of pulling the skin on its own, the surgeon works underneath the SMAS layer, releasing the ligaments that anchor sagging tissue in place and repositioning the skin, fat, and muscle of the face as one connected unit, in a vertical direction rather than sideways. Because the deeper structures of the face are actually restored to where they used to sit, rather than skin alone being pulled tight over sagging tissue underneath, the results tend to look softer and far more natural. Many patients find they hold up longer, too, often ten years or more, because the support comes from the tissue itself rather than from tension on the skin.
It is a more advanced, more technically demanding procedure. It is also why the surgeon performing it matters as much as the technique itself.
Why Santa Barbara and Los Angeles patients choose Dr. Keller
Dr. Gregory Keller has been performing facelifts since 1978 and has completed more than 5,000 deep plane facelifts, one of the most extensive track records of any facial plastic surgeon on the West Coast. He is double board certified, a Clinical Professor at UCLA, and Co-Director of the UCLA/AAFPRS Facial Plastic Surgery Fellowship, where he trains the next generation of facial plastic surgeons in the very technique he has spent his career refining.
Dr. Keller sees patients at his Santa Barbara office at 2305 De La Vina Street and his Los Angeles office at 10921 Wilshire Boulevard, so patients throughout Montecito, Santa Barbara, Ventura, and across Los Angeles can be seen close to home without compromising on who performs their surgery.
His approach is built around one goal: results that restore a natural, rested version of a patient’s own face, never a different face, and never a face that looks operated on.
“I feel prettier than ever”: one patient’s story
We hear stories like this often, and this one captures exactly why patients choose a deep plane facelift, and why they choose Dr. Keller to perform it.
I am 54 years old, and I feel prettier than ever!
My husband works with plastic surgeons across the country. When my sagging jawline could no longer be improved with minimally invasive procedures or endless filler, I told him I didn’t want to look different. I just wanted to look refreshed and look great for my age.
He told me the best facial plastic surgeon he knew was Dr. Keller.
I absolutely love my natural-looking deep plane facelift. My only regret is not doing it sooner.
What I didn’t expect was how much it would change the way I feel about myself. I look in the mirror and see a younger version of myself, and I carry myself like a younger version of myself too.
The compliments I receive about looking great for my age are wonderful, but what this has done for my self-esteem has been priceless. I truly believe the second half of my life is going to be the best half of my life.
Thank you, Dr. Keller!
Notice what she doesn’t say. She doesn’t say she wanted to look like someone else. She wanted to look like herself again, and to stop feeling like her reflection and her self-image were out of sync.
What life looks like on the other side
That shift in confidence is not unique to one patient. It is the pattern Dr. Keller hears again and again.
Patients describe saying yes again, to the dinner invitation, the reunion, the charity gala, the photo they used to dodge. Instead of hanging back at the edge of a room, they walk in and start the conversation themselves. The energy that used to go into hiding, into camera angles and concealer and avoiding certain lighting, goes somewhere else instead: into relationships, into showing up, into simply being present. Patients often describe it less as looking different and more as finally looking like how they already feel on the inside.
Recovery: what to expect
A deep plane facelift is real surgery, and it deserves a real recovery timeline, not a rushed one. Most patients see the majority of swelling and bruising resolve within two to three weeks, with sutures removed in the first week. Many feel comfortable in social settings again around the three to four week mark, often with the help of styling or light makeup in the early weeks. Results continue to refine and soften for several months afterward, and because the deeper facial structures have been repositioned rather than simply tightened, the outcome tends to hold up for years rather than months.
If you don’t like the way you’re aging, let’s talk
You do not need to have all the answers before reaching out. You do not need to already know if surgery is right for you. You just need to be honest that something has changed, that the mirror and the way you feel inside do not match anymore, and that you are curious what your options actually are.
Dr. Gregory Keller and his team see patients from Santa Barbara, Montecito, Ventura, and across Los Angeles who are simply ready to feel like themselves again. If that sounds like you, give us a call and let’s meet and talk about your options.
